Pharmacokinetics Later properties of intake is quickly absorbed in digestive tract. The bioavailability is 29-57%. Therapeutic effective concentration of drug in blood plasma make 0.2-2.0 mkg/ml. At repeated use of drug its pharmacokinetics does not change.
Contacts proteins of blood plasma for 90%. Easily passes through gistogematichesky barriers. In a liver and fatty tissue forms depot. Is exposed to biotransformation in a liver. The semi-elimination period (T½) a papaverine is 0.5-2.0 h. It is removed in the form of the phenolic metabolites conjugated with glucuronic acid with urine, insignificant quantities of a papaverine (less than 0.5%) are removed in not changed look.
The pharmacodynamics
the Papaverine has hypotensive and myotropic spasmolytic effect.
The mechanism of action is connected with ability of a papaverine to block activity of phosphodiesterase IV of type. Blockade of enzyme leads to the termination of hydrolysis of tsAMF and increase in its concentration in smooth muscle cells of vessels, internals. tsAMF limits receipt of calcium ions in a muscle cell and inactivates a kinase of light chains of myosin of sokratitelny protein which provides reduction of a muscle.
The papaverine reduces a tone and relaxes unstriated muscles of internals (digestive tract, airways, an urinogenital system) and vessels. The papaverine expands mainly arterial vessels and promotes increase in a blood-groove, including cerebral. Has hypotensive effect. In high doses reduces excitability of a cardiac muscle and slows down intracardial conductivity.
At use in average therapeutic doses the papaverine has practically no effect on the central nervous system.
Indications
– spasms of unstriated muscles of internals in cholecystitis, a spastic colitis, a pylorospasm (prevention and stopping)
– encephalopathies of vascular genesis (as a part of complex therapy)
Apply the Route of administration and doses inside on an empty stomach. The adult appoint for 0.04-0.06 (1-1½, tablets) 3-5 times a day. The highest single dose of 0.4 g (10 tablets), maximum daily dose of 0.60 g (15 tablets). Duration of a course of treatment is 10-14 days, if necessary carrying out repeated courses of use of drug at an interval of 7-10 days is possible.
At persons 70 years an initial single dose no more than 0.01 g are more senior (& frac14, tablets).
To children from 10 years appoint in a dose 0.3-0.5 mg/kg of weight 2-3 times a day.

Medicinal interactions
Reduces protivoparkinsonichesky effect of a levodopa and hypotensive effect Methyldopums. In a combination with barbiturates the spasmolytic action of a papaverine of a hydrochloride amplifies. At combined use with tricyclic antidepressants, procaineamide, reserpine, quinidine sulfate strengthening of hypotensive effect is possible.
Special instructions
It is necessary to be careful and use only small doses of a papaverine at persons with a craniocereberal injury, abnormal liver functions and kidneys, a hypothyroidism, insufficiency of function of adrenal glands as they strengthen hypotensive action of a papaverine. At persons with a benign hyperplasia of a prostate the high doses of a papaverine can provoke development of a sharp ischuria.
During use of a papaverine of a hydrochloride it is necessary to exclude the use alcohol – the containing drinks and also the medicines containing alcohol.
Use in pediatrics
Use of tablets of a papaverine of a hydrochloride for children up to 10 years is not recommended.
Pregnancy and a lactation
At pregnancy and in the period of a lactation safety and efficiency of use of a papaverine of a hydrochloride are not established.
Features of influence of medicine on ability to run the vehicle or potentially dangerous mechanisms
Considering side effects of drug it is necessary to be careful at control of motor transport or other mechanisms.
Overdose
Symptoms: diplopia, weakness, drowsiness, hypotension.
Treatment: specific antidote does not exist. Measures of the help include drug withdrawal, gastric lavage with milk and activated carbon and the symptomatic therapy directed to elimination of the arisen disturbances and increase in arterial blood pressure.
