Pharmacological

Tobramycinum is emitted quickly and extensively in urine by glomerular filtration, generally as not changed medicine. T1/2 from plasma of about 2 h with clearance of 0.04 l/hour/kg and of distribution of 0.26 l/kg. Binding of proteins of plasma Tobramycinum less, than 10%. Oral bioavailability of Tobramycinum (‹1%).
A pharmacodynamics
the Antibiotic of a broad spectrum of activity from group of aminoglycosides. In low concentration works bacteriostatically (blocks 30S a subunit of ribosomes and breaks protein synthesis), and in higher – bakteritsidno.
It is highly active concerning Staphylococcus spp. (including Staphylococcus aureus, Staphylococcus epidermidis/coagualase - negative and a coagualase - positive / and also the strains resistant to penicillin), Streptococcus spp. (including and-b-hemolytic types, some not hemolytic types, Streptococcus pneumoniae), Pseudomonas aeruginosa, Escherichia coli, Klebsiella pneumoniae, Serratia spp., Providencia spp., Shigella spp., Citrobacter spp., Enterobacter aerogenes, Proteus mirabilis (an indole - negative) and an indole - positive types of Proteus spp., Hemophilius influenzae, Hemophilius aegyptius, Moraxella lacunata, Acinetobacter calcoaceticus (Herellea vaginacola), some types of Neisseria spp. (including Neisseria gonorrhoeae).

Indications
Treatment of the superficial bacterial infections of an eye caused by activators, sensitive to Tobrimed:
- blepharitis
- conjunctivitis, a keratoconjunctivitis, blefarokonjyunktivit
- a keratitis
- an entophthalmia
- a dacryocystitis
- a meibomitis
Prevention of postoperative infectious complications in ophthalmology.

A route of administration and doses
Only for topical administration.
The strip of ointment about 1 cm long is put for a lower eyelid by 2-3 times a day.
At a sharp course of an infection ointment is appointed each 3-4 hours, in process of weakening of symptoms of a disease reduce drug use frequency. It is not necessary to touch with a tuba tip tip eyes or any other surface to avoid pollution of contents of a tuba.
After putting drug the pressing of the lacrimonasal channel or cover is recommended a century. It reduces system absorption and an adverse effect of the medicine which is applied locally through eye tissues.
Course of treatment of 7 days.

Side effects
Local

Contraindications
- hypersensitivity to Tobramycinum or other aminoglycosides, or to any other component of drug.

Medicinal interactions
in case of co-administration of Tobrimed oculentum with system antibiotics from group of aminoglycosides strengthening of system side effects is possible (nephrotoxic, ototoksichesky action, disturbance of mineral exchange and a hematopoiesis).

Special instructions
it is not recommended to carry contact lenses during Tobrimed's use. It is necessary to avoid prolonged use because of danger of reproduction of microorganisms, insensitive to drug.
Use in pediatrics
the Efficiency and safety of use of Tobrimed for children aged up to 2 months is not studied.

Tobrimed pregnant women should appoint pregnancy and the period of a lactation only in case of emergency when the efficiency of treatment justifies possible risk for a fruit.
For feeding by a breast it is necessary to stop use of Tobrimed oculentum at use of high doses or long treatment.
Contact lenses
for treatment by Tobrimed, ophthalmologic ointment it is necessary to refrain from carrying soft (hydrophilic) contact lenses. Lenses can be carried in breaks between use of drug and to insert not earlier than in 15 minutes after ointment use. It is impossible to apply Tobrimed over contact lenses.
Features of influence of medicine on ability to run the vehicle or potentially dangerous mechanisms
After putting ointment are possible temporary ambiguity of sight or other visual concerns that can negatively affect ability to drive the car or other potentially dangerous mechanisms

Overdose
Symptoms: a dot keratitis, an erythema, the raised dacryagogue, an itching and hypostasis of a century.
Treatment: symptomatic therapy.
