Pharmacokinetics Later properties of drawing on the uninjured skin under an occlusive bandage is soaked up on average with a speed of 0.2-2.8 mm/h. System absorption is less than 0.1%. In 8 hours after applique of 8 g of ointment under an occlusive bandage the camphor level in blood plasma is 41.0±5.8 ng/ml and is reached in 5.6±1.3 h, and is reached menthol of 31.9±8.8 ng/ml in 4.7±5.4 h. At applique in a dose of 4 g the levels of camphor and menthol are 26.8±7.2 ng/ml and 19.0±5.4 ng/ml respectively. After applique in number of 2 g camphor and menthol in system circulation are not defined. The part of terpenic components of ointment which underwent absorption is oxidized further the system of cytochromes of a liver with formation of hydroxy derivatives which then conjugate with glucuronic acid and are allocated with urine (41.1-47.7%) and secretion of mucous glands. In not changed look less than 0.014% of all dose of the ointment applied on the surface of skin are allocated.
Ointment gevkamen has expressed the local irritative and distracting effect, weak local anti-inflammatory action.
Action of a gevkamen is caused by the content of essential oils and terpenic derivatives (camphor and menthol) which are a part of ointment. The main operating components of clove oil are eugenol (60-95%), its acetic ether (2-27%) karyophyllenes (5-10%). The main operating components of eucalyptus oil are 1.8 cineole (54-95%), α-pinene (2.6%), r-tsimen (2.7%). Terpenic derivatives stimulate cold and pain receptors in a contact zone, rendering irritant action. Eugenol in concentration of 3.3-16.4 mkg/ml and components of essential oil of an eucalyptus in concentration of 5.8 mkg/ml oppress activity of cyclooxygenase I and II of types and also lipoxygenases. Reactions of an arachidonic cascade of synthesis of eicosanoids are as a result blocked and formation of prostaglandins, thromboxane and leukotrienes in the inflammation center is broken.
Right after applique of ointment its irritant action in the form of burning sensation, hyperaemia and hypostasis in the site of application of medicine is shown. It is connected as with direct activation by the irritating components of ointment of pain receptors in fabric, and with discharge under their influence of mediators of inflammation (histamine, serotonin, bradykinin) from depot. These mediators increase sensitivity of receptors to painful incentives and cause vasodilation at the expense of what the blood stream in vessels of soft tissues and skeletal muscles amplifies, regeneration processes are stimulated. Anti-inflammatory and analgeziruyushchy effect of ointment is shown approximately in 0.5 h after applique.
Applique on frontotemporal area and juxtaspinal zones of a neck promotes relaxation of muscles of a neck and reduces severity of a headache of tension, headaches of cluster type in 15 min. after drawing.
At local drawing weakens cancerogenic action on skin of benzpyrenes.
– as a part of complex therapy in degenerative diseases of a backbone: osteoarthroses, osteoarthropathies
– posttraumatic inflammations of sinews, ligaments, muscles and joints (stretchings, sports injuries, overloads, bruises)
– neuralgia, myalgias, arthralgias
– a headache of tension and a cluster headache.
The route of administration and doses
apply Drug on the clean dry surface of the uninjured skin. Ointment is evenly distributed on the surface of skin in the field of the painful site then slightly mass a hand the site of application of ointment before emergence of feeling of easy burning or a cold snap. After applique it is necessary to wash up hands (if only this area is not exposed to treatment).
To adults and children 12 years are more senior appoint depending on the size of a painful zone (body). On the site of 400-800 cm2 (corresponds to the surface of a knee joint and a sacrum) it is necessary to apply 2-4 g ointments (corresponds to an ointment strip of 2-4 cm). Appliques repeat 3-4 times a day. The maximum daily dose – 8 g (an ointment strip of 8 cm). Duration of a course of treatment is 3-5 days, if necessary courses are repeated at an interval of 2-3 days.
– skin allergic reactions (burning, hyperaemia), an itching in the field of applique
– irritation of skin, eczema
– children’s age up to 12 years
– hypersensitivity to essential oils of mint, a clove tree, an eucalyptus, plants of family myrtaceous.
Due to the low system absorption of ointment gevkamen risk of development of medicinal interactions is low.
Nevertheless it is necessary to remember that the terpenic components which are a part of drug are inductors of the microsomal enzymes of a liver metabolizing other medicines. Therefore there is a probability of easing of effect of other medicines applied together with gevkameny.
At simultaneous use with non-steroidal anti-inflammatory drugs for topical administration enhances their effect and increases penetration depth in soft tissues in an applique zone.
The dimethyl sulfoxide at simultaneous applique with ointment gevkamen strengthens system absorption of the components of essential oils which are its part and increases penetration depth in soft tissues in an applique zone.
instructions Pregnancy and a lactation
Despite the minimum system absorption of a gevkamen the use of ointment during pregnancy and a lactation is possible only after careful assessment of the expected advantage for mother and possible risk for the child or a fruit.
Ointment of a gevkamen is intended to feature of use only for external use. Drug is recommended to be applied only to the uninjured skin, avoiding hit of ointment on sites of damage and open wounds. It is not necessary to allow hits of drug on a face, mucous membranes of a mouth, an eye, vaginas and a rectum.
Influence on ability to driving of motor transport and control of mechanisms
Ointment gevkamen does not influence ability to driving of motor transport or control of mechanisms and other operator activity.
Overdose of a gevkamen at its topical administration in the form of ointment is improbable that is connected with its low system absorption. The likelihood of development of overdose raises at accidental or deliberate use of ointment inside (during 1981-1992 only 109 cases of intoxication of this sort are described). The typical clinical picture at the use of a gevkamen inside does not develop.
Possible symptoms: dizziness, heartburn, nausea, vomiting, bradycardia and decrease in the ABP, disorientation, confusion of consciousness, muscle weakness, miosis, spasms.
Measures of the help include gastric lavage (in case of intake of ointment inside), the supporting and symptomatic therapy directed to elimination of the arisen disturbances. Specific antidote does not exist.
