Pharmacological properties

Pharmacokinetics

Metronidazole is well absorbed when administered intravaginally in the form of tablets. Metronidazole is distributed into most tissues and body fluids. It also penetrates through the placental barrier and into mother’s milk. The biological half-life in adults with normal kidney and liver function is 6-8 hours. In patients with impaired liver function, the half-life of metronidazole may increase. Metronidazole is metabolized in the liver. Metronidazole and its metabolites are excreted primarily in the urine. Excreted by 40 – 70% (about 20% in unchanged form) through the kidneys. The drug can be removed from the body by hemodialysis.

Pharmacodynamics

Trichopol is an antimicrobial drug, belongs to nitro-5-imidazoles and is a DNA-tropic drug with a bactericidal type of action.

The mechanism of action of metronidazole is the biochemical reduction of the 5-nitro group of metronidazole by intracellular transport proteins of anaerobic microorganisms and protozoa. The reduced 5-nitro group of metronidazole interacts with the DNA of microorganism cells, inhibiting the synthesis of their nucleic acids, which leads to the death of microorganisms.

Metronidazole is a broad-spectrum antimicrobial and antiprotozoal agent. The drug is highly active against Trichomonas vaginalis Giardia intestinalis Entamoeba histolytica Lamblia intestinalis Balantidium coli Blastocystis hominis Helicobacter pylori ; in relation to obligate anaerobes and (spore- and non-spore-forming) – Bacteroides spp (V. fragilis , V. ovatus , V. distasonis V. thetaiotaomicron , B. vulgatus ), Fusobacterium spp ., Clostridium spp ., Peptostreptococcus spp ., Peptococcus spp , susceptible strains of Eubacterium ).  

Metronidazole is active against the facultative anaerobic bacterium Gardnerella vaginalis .

 

Indications for use

– trichomonas and nonspecific vaginitis

– bacterial vaginosis

– in the complex treatment of inflammatory diseases of the pelvic organs (endometritis, pelvioperitonitis, salpingitis, tubovarial abscess) as part of complex therapy

 

Dosage and administration

One vaginal tablet per day for 7-10 days.

Treatment with metronidazole should not last more than 10 days and be repeated more than 2-3 times a year.

Having previously released the tablet from the contour packaging with scissors (cut the film along the contour of the tablet), you can moisten the tablet with boiled chilled water and insert it deep into the vagina.

 

Possible side effects

– itching, burning, pain and irritation in the vagina; thick, white mucous discharge from the vagina (odorless or with a slight odor), frequent urination, after discontinuation of the drug – the development of vaginal candidiasis

– burning sensation or irritation of the penis in the sexual partner

Nausea, change in taste sensations, metallic taste in the mouth, dry mouth, decreased appetite, abdominal cramps, nausea, vomiting, constipation or diarrhea

– allergic reactions: rash, itching of the skin, rash

– headache, dizziness

– leukopenia or leukocytosis

Rarely

 staining of urine in a red-brown color, due to the presence of a water-soluble pigment formed as a result of the metabolism of metronidazole

 

Contraindications

– hypersensitivity to metronidazole or other nitroimidazole derivatives

– blood diseases, leukopenia (including history)

– violation of coordination of movements, organic lesions of the central nervous system (including epilepsy)

– liver failure (in case of high doses)

– 1st trimester of pregnancy

 

Drug Interactions

Compatible with sulfonamides and antibiotics. When treating with metronidazole, alcohol intake should be avoided (similar to disulfiram causes alcohol intolerance). You should not combine metronidazole with disulfiram, since the interaction of these drugs may cause depression of consciousness, the development of mental disorders.

When used simultaneously with warfarin and other indirect anticoagulants, metronidazole enhances their effect, which leads to an increase in prothrombin time. It is not recommended to combine with non-depolarizing muscle relaxants (vecuronium).

Under the influence of barbiturates, a decrease in the effect of metronidazole is possible, since its inactivation in the liver is accelerated.

Cimetidine inhibits the metabolism of metronidazole, which can lead to an increase in its concentration in the blood serum and an increase in the risk of adverse reactions.

When taken simultaneously with lithium preparations, the concentration of the latter in plasma may increase.

 

Special instructions

In the case of trichomoniasis, simultaneous treatment of the sexual partner with the drug is recommended, regardless of whether he has manifestations of the disease.

During the period of drug treatment, it is recommended to refrain from sexual intercourse.

Treatment with metronidazole should not last more than 10 days and be repeated more than 2-3 times a year.

With indications in the anamnesis of changes in the composition of peripheral blood, as well as when using the drug in high doses and / or with its long-term use, it is necessary to control the complete blood count.

Metronidazole can immobilize treponema, resulting in a false positive TPI test (Nelson test).

Pregnancy and lactation

The drug is used with caution from the second trimester of pregnancy.

Metronidazole passes into breast milk. Breastfeeding during the period of taking the drug should be canceled. Breastfeeding should be resumed no earlier than 48 hours after the end of the drug.

Influence on the ability to drive vehicles and maintain moving mechanisms

The drug can cause headaches and dizziness, which can have a negative effect on driving a vehicle and maintaining moving mechanisms.
