Structure

active agent – a terbinafin a hydrochloride of 10 mg
of equivalent 8.888 mg of a terbinafin,
excipients: sodium hydroxide, benzyl alcohol, sorbite
monostearate, cetyl palmitate, cetyl alcohol, stearyl alcohol, polysorbate 60, isopropyl myristate, water purified.

Pharmacokinetics At properties topical administration of TERFALIN cream ® is soaked up less than 5% of a dose and presence of a terbinafin at system blood circulation minimum. Terbinafin quickly gets into a thermal layer of skin and collects in a lipophilic corneal layer. If duration of treatment lasts from 1 to 7 days, elimination half-life of a terbinafin from a corneal layer raises by 3 times.

TERFALIN pharmacodynamics ® is to synthetic derivatives of allylamine which has the broad antifungal spectrum of action
of TERFALIN ® in unusual way changes an early stage of the biosynthesis of sterols happening in mushrooms. It leads to deficiency of ergosterol and to intracellular accumulation of squalene that causes death of a cell of a mushroom. Terbinafin has the fungicide effect concerning dermatophytes, yeast-like mushrooms and some dimorphous mushrooms.
TERFALIN ® shows activity against the majority of strains of Trichophyton mentagrophytes, Trichophyton rubrum.
Cream of 1% of TERFALIN ®проявляет satisfactory MOC (minimum overwhelming concentration) against the majority of strains of Candida albicans, Epidermophyton floccosum, Scopulariopsis breviacuaulis.
At external use terbinafin it is active also concerning Pityrosporum orbiculare.

Indications
– the mycoses of smooth skin and large folds caused by Trikho’s strains
of a fiton of T. rubrum and T mentagrophytes, M. Canis, Epidermophyton floccosum
– mycoses of feet (the athlete’s foot) and brushes
– skin candidiasis, caused by the sort Candida (including Candida albicans)
– multi-colored deprive the caused Pityrosporum orbiculare (also izvest
ny under the name Malassezia furfur).

The route of administration and doses
of TERFALIN ® cream of 1% is applied one or twice a day depending on indications. Duration of treatment varies according to indications and weight of an infection. Before use the struck surface of skin, it is necessary to wash out and to dry up carefully. In the infections which are followed by an intertrigo (under mammary glands, in interdigital intervals, between buttocks, in inguinal area), sites of application of cream are recommended to be covered with a gauze, especially for the night.
The recommended treatment duration:
Mycoses of smooth skin and large folds: 1 – 2 weeks

Mycoses of feet and brushes: 1 week (twice a day), 2 weeks (once a day)
skin Candidiasis: 1 –
Multi-colored deprive 2 weeks: 2 weeks.
Clinical improvement occurs within several days. Irregular use or the early termination of treatment terbinafiny can increase risk of repeated infection. In case in 2 weeks signs of improvement are not noted, it is necessary to carry out verification of the diagnosis.
Dose adjustment is not required from elderly patients.

The special
instructions Cream it is intended only for external use, and not intended for intake, intranasal use, use in ophthalmologic practice and intravaginalny use.
With care: liver and/or renal failure, alcoholism, oppression of a marrowy hemopoiesis, tumor, metabolism disease, occlusal diseases of vessels of extremities.
It is necessary to avoid hit of cream on a mucous membrane of eyes. At hit of cream in an eye it is necessary to wash out their large number waters and, if necessary, to consult with the doctor.
In cases of irritation or allergic reactions or deteriorations in health it is necessary to stop use of a terbinafin and the corresponding treatment has to be appointed.
The infected sites should not be fitted by close clothes.
Patients with an infection of legs have to wear well adjusted and ventilated shoes. It is necessary to change footwear and socks, at least, once a day.
Observance of the recommended treatment duration is important for achievement of the best clinical effect.
It is necessary to wash hands after each use for prevention of planting from the infected sites.
Pregnancy and a lactation
of Adequate and well controlled tests on pregnant women it was not carried out. During pregnancy and a lactation the use is possible only according to strict indications.

Drug does not affect ability to run the vehicle or potentially dangerous mechanisms.

The overdose
Is possible only at accidental intake. Symptoms: a headache, dizziness, nausea, pain in epigastriums. Treatment: use of activated carbon and symptomatic and maintenance therapy.
