Pharmacological action
Pharmacokinetics
At intake is quickly absorbed from digestive tract. At single dose in a dose of 200 mg the peak of serumal concentration (9.48 mg/l) is reached in 2 hours and so quickly brought out of an organism (the half-life period is equal to 2 hours 45 minutes). Nifuratel has high bioavailability (& gt, 95%). Gets through a blood-brain and gematoplatsentarny barrier, it is allocated with breast milk. It is metabolized in a liver and muscle tissue.

The pharmacodynamics
Nifuratel, derivative nitrofuran, is synthetic antibacterial drug of a broad spectrum of activity. Drug highly – is effective concerning bacterial, protozoan and fungal activators. The mechanism of action is caused by blockade of process of polymerization of a peptide chain and, therefore, suppression of synthesis of DNA in bacterial cells, sensitive to drug. The inhibition of synthesis of protein in ribosomes of a bacterial cell occurs due to irreversible linking with S30 and S50 subunits of ribosomes therefore process of forming of all peptide chain is broken.
Besides, there are data that nifuratet has also bactericidal effect. The mechanism of such action is connected with recovery of nitrogroup in an amino group. Indications

– pyelonephritis, an urethritis, cystitis, a pyelitis and other diseases of an urinary system
– an intestinal amebiasis and a giardiasis
– the chronic inflammatory diseases of upper parts of digestive tract caused by Helicobacter pilory.
The route of administration and doses
take the Drug inside, following recommendations.
Vaginal infections:
Adults: 1 tablet 3 times a day after a meal within 7 days (both sexual partners have to take the drug)
Children since 6 years: the recommended dose of 10 mg/kg of body weight daily within 10 days. The recommended dose has to be accepted in two steps.
Infections of urinary tract:
Adults: depending on weight of a course of the disease of 3-6 tablets after meal daily within 7-14 days.
Children since 6 years: the recommended dose of 30-60 mg/kg of body weight. The recommended dose has to be accepted in two steps.
According to the recommendation of the doctor the course of treatment of infections of urinary tract can be prolonged or repeated.
Intestinal amebiasis:
Adults: 2 tablets 2-3 times a day within 10 days.
Children since 6 years: the recommended dose of 10 mg/kg of body weight 3 times a day.
Giardiasis:
Adults: 2 tablets 2-3 times a day within 7 days.
Children since 6 years: the recommended dose – 15 mg/kg of body weight 2 times a day within 7 days.
The GIT infections associated with Helicobacter pylori:
Adults: 2 tablets 2-3 times a day within 7 days.
Children since 6 years: the recommended dose of 15 mg/body weight 2 times a day within 7 days.
Side effects
– nausea, vomiting, bitterness in a mouth, diarrhea, heartburn, a gastralgia
– skin rash, an itching.
Contraindications
– hypersensitivity to active agent or any component of drug
– the children’s age up to 6 years
Medicinal interactions
Strengthens fungicide effect of nystatin.
Special instructions
need to be carried out simultaneous treatment of the sexual partner because of danger of repeated infection. At treatment of vaginal infections only performing oral therapy by the drug Makmiror recommends to increase a day dosage to 4 – 6 tablets.
During treatment it is necessary to refrain from sexual contacts or to apply in addition Makmiror before each sexual intercourse.
In order to avoid strengthening of side effects it is not recommended to apply alcoholic beverages during the therapy period the drug Makmiror. Use at pregnancy and a lactation
Nifuratel gets through a blood-brain barrier therefore use of drug is possible only according to strict indications if the expected effect of therapy exceeds potential risk for a fruit.
Nifuratel is excreted in breast milk therefore in need of prescribing of drug in the period of a lactation it is necessary to resolve an issue of the breastfeeding termination.
