Structure
1 g of ointment contains
active agents: heparin sodium (at activity of ME/mg heparin sodium 120) – 100 ME (0.833 mg)
benzocaine – 40 mg
benzilnikotinat – 0.8 mg
excipients: glitserol, vaseline, stearic acid (stearic acid 50), a peach oil, emulsifier No. 1, methylparahydroxybenzoate, propilparagidroksibenzoat, the water purified.

Pharmacotherapeutic group

drugs ATX C05BA53 Code Pharmacological Pharmacokinetics Heparin Properties is slightly soaked up from the surface of skin. Communication with proteins of plasma – up to 95%, distribution volume – 0.06 l/kg. Does not get through a placenta and into breast milk. It is intensively taken endothelial cells and cells of a mononuklearno-macrophagic system, concentrates in a liver and a spleen. It is metabolized in a liver with participation of N-desulfamidazy and haparinase of thrombocytes joining in metabolism of heparin at later stages. Desulfatirovanny molecules under the influence of endoglycosidase of kidneys turn into low-molecular fragments. Elimination half-life (T1/2) – 1-6 h (on average – 1.5 h), increases in obesity, a liver and/or renal failure, decreases in a pulmonary embolism, infections, malignant tumors. It is removed by kidneys, mainly in the form of inactive metabolites. It is not removed by means of a hemodialysis.
Benzilnikotinat gets into all layers of skin. Practically does not come to a system blood stream.
Benzocaine at external use is practically not absorbed.
A pharmacodynamics
the Combined drug for external use which action is caused by properties of the components which are its part. Heparin – anticoagulant of direct action, belongs to group of middlemolecular heparins. At external use renders local anticoagulating, antiexudative, moderate anti-inflammatory action. Blocks formation of thrombin. Heparin reduces inflammatory process and has anticoagulating effect, indirectly improves microcirculation that promotes process of resorption of hematomas and blood clots and reduction of puffiness of fabrics.
Benzilnikotinat is peripheral vazodilatiruyushchy means, promotes microcirculation improvement. It is used as adjuvant for heparin penetration improvement.
Benzocaine has the local anesthetizing effect. Reduces permeability of a cellular membrane for sodium ions, forces out calcium ions from the receptors located on an internal surface of a membrane blocks carrying out nervous impulses. Interferes with emergence of painful impulses in the terminations of sensory nerves and to their carrying out on nerve fibrils.
Indications
– prevention and treatment of thrombophlebitis of superficial veins
– post-injection and post-infusional phlebitis
– external hemorrhoids, inflammation of postnatal hemorrhoidal nodes
– a limfedem (elephantiasis)
– the superficial periphlebitis
– limfangiit
– superficial mastitis
– the localized infiltrates and hypostases
– injuries and bruises without disturbance of integrity of integuments (including muscle tissue, sinews, joints)
– a hypodermic hematoma.
Route of administration and doses
Outwardly. Ointment inflict a thin layer on area defeats from calculation 0.5-1g over ointment (2-4 cm of ointment) on the site with a diameter of 3-5 cm and carefully rub in skin. Ointment is applied by 2-3 times a day daily before disappearance of the inflammatory phenomena, on average from 3 to 7 days. The possibility of performing more long-term treatment is defined by the doctor.
In thrombosis of external hemorrhoidal nodes ointment is applied on byazevy or linen laying which is imposed directly on the thrombosed nodes and fixed. With the same purpose apply the rectal tampons impregnated with Unguentum Heparini which enter into an anus. Ointment should be applied daily before disappearance of symptoms, on average from 3 to 14 days.

Medicinal interactions
do not appoint Ointment locally along with non-steroidal anti-inflammatory drugs (NPVP), tetracyclines, H1 blockers – histamine receptors.
Special instructions
At long use of Unguentum Heparini on extensive sites and a concomitant use of the oral medicines influencing the coagulant system of blood and/or a hemopoiesis it is necessary to control time of coagulability and a prothrombin time.
In need of a long course of treatment and at extensive sites of defeat at pregnant women and at women 65 years are aged more senior similar control is recommended.
It is not necessary to apply on open wounds, in the presence of purulent processes.
Use of ointment is not recommended in deep venous thrombosis.
Pregnancy and a lactation
Safety of use of drug at pregnancy and in the period of a lactation is not established. Heparin does not pass through a placental barrier and is not emitted with breast milk. There are no data on hit of other components of drug in breast milk.
Unguentum Heparini is applied at pregnancy and in the period of a lactation it is only strict according to indications.
The feature of influence of medicine on ability to drive the car or potentially dangerous mechanisms
does not influence ability to drive the car or potentially dangerous mechanisms.
The overdose
Due to the insignificant system absorption overdose is improbable.
The form of release and packing
On 25 g of drug place aluminum or polyethylene laminatny in tubas.
Each tuba together with the instruction for medical use in the state and Russian languages is placed in a pack from cardboard.
To Store storage conditions at a temperature not above 20 °C.
