Pharmacokinetics Iron properties in the drug Ferrum Lek® chewable tablets contains in a type of a complex of ferrous hydroxide (III) and a polymaltose in which its particles are built in molecules of polymeric carbohydrate (polymaltose). In such look iron does not make harmful effects on digestive tract and does not enter interaction with food. At the same time also the bioavailability of iron increases. The iron absorption determined by hemoglobin level in erythrocytes in inverse proportion to the entered dose (the dose is higher, the absorption level is lower). The structure of a complex of ferrous hydroxide (III) and polymaltose is similar to the structure of the ferritin natural of the iron-reserving protein which is contained in an organism. Thanks to this similarity, iron (III) from this complex is soaked up by means of process of active absorption. The highest level of absorption is noted in duodenal and lean guts. The soaked-up iron generally stocks up in a liver in a complex with ferritin, then in marrow its inclusion in composition of hemoglobin is carried out. Not soaked up iron is removed with excrements. The amount of the iron which is brought out of an organism together with the exfoliated cells of an epithelium of digestive tract and skin also with then bile and urine, is about 1 mg a day. At women it is necessary to take into account loss of iron with menstrual blood.
A pharmacodynamics
Multinuclear molecules of a giroksid of iron (ІІІ), in the drug Ferrum Lek®, are surrounded not covalently with the related molecules of a polymaltose, forming a complex with a molecular weight about 50 cd which thanks to the sizes, in comparison with bivalent iron (ІІ), passes through a membrane of mucous membranes of intestines by means of the mechanism of simple diffusion 40 times more weakly. The complex is stable, in physiological conditions, iron ions from it are not released.
In the drug Ferrum Lek® a ferrous hydroxide complex (ІІІ) and the polymaltose has no pro-oxidatic properties, inherent in salts of bivalent iron. The sensitivity of lipoproteins of very low and low density to the oxidizing factors in this regard decreases.
The iron which is a part of drug is deposited in a liver in the form of a complex with ferritin, then is included hemoglobin, that quickly filling shortage of iron and restoring hemoglobin level in an organism. At drug treatment there is a gradual regression clinical (weakness, fatigue, dizziness, tachycardia, morbidity and dryness of integuments) and laboratory symptoms of deficiency of iron.

Ferrum of Lek® is recommended to take chewable tablets in time or right after food. It is possible to chew or swallow of chewable tablets entirely. The daily dose can be divided into several receptions or to take for 1 time.
The iron deficiency anemia with clinical manifestations
Children is more senior than 12 years, adult, nursing mothers
1-3 Lek® Ferrum chewable tablets a day. Duration of treatment is 3-5 months before normalization of level of hemoglobin. After that administration of drug should be continued within several weeks for completion of reserves of iron in an organism.
A latent iron deficiency anemia
1 chewable tablet a day. Treatment continues within 1-2 months.
Pregnant women
the Iron deficiency anemia with clinical manifestations
2-3 chewable tablets a day before achievement of normalization of size of hemoglobin. After that therapy has to continue with 1 chewable tablet Ferrum of Lek® in day, at least, before childbirth for completion of reserves of iron.

Side effects
Ferrum Lek® as a rule, is well transferred by patients. Mostly, undesirable effects were easy and passing.
Very seldom

– urticaria, rash, a dieback,

– the anemia which is not caused zhelezodefitsity (hemolytic anemia)

Medicinal interactions
Still interactions were not observed. As iron is connected with a complex, the probability that there will be an ionic interaction to food components (phytin, oxalates, tannin, etc.) and at the same time taken medicine (tetracyclines, antacids) is small. The test for detection of the occult blood (selective for Hb) is not violated and, therefore, there is no need for therapy interruption by iron.

The special
instructions Drug does not cause coloring of enamel of teeth. Dosage forms of drug Ferrum of Lek® the taken inside and other iron preparations can paint kcal in dark color. It has no clinical importance.
To children aged up to 12 years due to the need of prescribing of drug in low doses the use in the form of syrup is preferable. In cases of the anemia caused by an infectious or malignant disease, iron collects in a reticuloendothelial system from which it will be mobilized and utilized only after treatment of a basic disease. When prescribing drug the patients with diabetes should consider that one tablet contains 0.04 grain units. Tabletki Ferrum Lek® contain aspartame, a phenylalanine source (predecessor of phenylalanine), in the quantity equivalent to 1.5 mg on 1 tablet that can cause adverse influence on patients with phenylketonuria. Patients with rare hereditary problems of intolerance of fructose, malabsorption of glucose galactose or insufficiency of invertase-isomaltase should not take this medicine.
Pregnancy and the period of a lactation
drug Use Ferrum Lek®, chewable tablets during pregnancy and a lactation is possible only after consultation with the doctor. Advantage/risk of use of drug is recommended to estimate a ratio.
